• Insurance claim forms
• Prescriptions for taking medication abroad
• Private sick notes
• Vaccination certificates

The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.

A self certificate is required for absence from work of less than seven days duration. Your employer should be able to provide this certificate. If you are off for seven or more days, you will need to speak to  your doctor, to get the appropriate certificate.

We work closely with our Health Visitor to ensure children complete a specific programme of child health development checks and immunisations by age 5.
